What can we learn from this case?

  • The operator is improperly trained and overestimate self-potential. His case selection & treatment was totally wrong
  • Lack of knowledge in Periodontology. Treating a patient over the age of 30, the risk of horizontal alveolar bone loss is great as the orthodontic force is applied and regeneration power is decreased
  • Treating a patient over the age of 30, the tooth extraction must be avoid.
In this case the extraction pattern was 14, 24 , 36 and 45, which can never be a proper pattern, because it will send overloading to the TMJs
  • The rule of extraction was not taken into consideration and here are “Symmetrical and Balancing Extraction in the same segment”
  • Optimal interincisal angle was not settle up. Vertical control was not performed. The bite become deeper and deeper with time
  • Finishing occlusion will be even more than worst. The operator was totally underestimate the side effects of closing 10 mm on the left side and 7 mm on the right side space in the mandible
  • The result of the treatment is nice smile but the patient cannot properly chew
  • TMD is invited
